What Is Nail Lichen Planus?
Nail lichen planus is a chronic autoimmune inflammatory condition that affects the nail matrix, nail bed, or both. In some patients, nail changes are the first or even only sign of lichen planus, making diagnosis challenging. Nail lichen planus can appear as thinning of the nail plate, ridging, nail lifting (onycholysis), and scarring at the area where the nail meets the cuticle. Because these findings closely resemble other nail disorders, many patients are mistakenly diagnosed and undergo months or even years of unnecessary treatment. Lichen planus can be aggressive and permanently destructive to the nail and so early, accurate diagnosis is essential.
What Causes Nail Lichen Planus?
Nail lichen planus, an autoimmune inflammatory disease in which the immune system mistakenly targets the healthy nail unit, can cause inflammation of the nail matrix, the nail bed, or both. If your nail lichen planus has not improved despite topical medications, steroid injections, oral medications, or repeated treatment attempts, Dr. Stern will carefully review your diagnosis, treatment history, and response to previous therapies to determine why improvement has been limited. Treatment failure may occur because nail lichen planus was mistaken for another condition, a fungal infection was present at the same time, the medication did not adequately reach the site of inflammation, the wrong treatment was used, or the treatment was not continued long enough to allow healthy nail growth.
