Nail Lichen Planus

Nail lichen planus, a chronic, autoimmune inflammatory disorder can affect both the fingernails and toenails. Lichen planus is also a skin disorder and can affect the scalp, oral cavity and genital region. Because early stage nail lichen planus often resembles brittle nails, an accurate diagnosis by a nail expert is essential. Now you can get expert nail care from one of the only nail dermatologists in the country with an easy-to-access telehealth appointment.

What Is Nail Lichen Planus?

Nail lichen planus is a chronic autoimmune inflammatory condition that affects the nail matrix, nail bed, or both. In some patients, nail changes are the first or even only sign of lichen planus, making diagnosis challenging. Nail lichen planus can appear as thinning of the nail plate, ridging, nail lifting (onycholysis), and scarring at the area where the nail meets the cuticle. Because these findings closely resemble other nail disorders, many patients are mistakenly diagnosed and undergo months or even years of unnecessary treatment. Lichen planus can be aggressive and permanently destructive to the nail and so early, accurate diagnosis is essential.

What Causes Nail Lichen Planus?

Nail lichen planus, an autoimmune inflammatory disease in which the immune system mistakenly targets the healthy nail unit, can cause inflammation of the nail matrix, the nail bed, or both. As a result, a wide variety of nail changes can be seen with this condition. In some individuals, nail disease may be the only manifestation of lichen planus, making diagnosis particularly difficult. Oftentimes the condition is idiopathic meaning that the cause is unknown.

When Should You Seek an Evaluation for Nail Lichen Planus?

If you’re experiencing any of the following nail changes, an evaluation may be warranted.

  • Nail ridging throughout many nails(sudden onset)

  • Nail lifting from the nail bed (onycholysis)

  • Thinning of the nail plate

  • Scarring of the nail causing nail deformity

  • Pain or tenderness around the nail

  • Changes affecting one or multiple nails

  • Psychological distress or embarrassment related to nail appearance

How Dr. Stern Evaluates Nail Lichen Planus Virtually

For both asynchronous e-visit and live video consults, Dr. Dana Stern will review your photos and intake forms, prior treatments and your current medications and health history. For asynchronous e-visits, you will receive a detailed treatment plan to share with your local healthcare provider, including recommendations for imaging, procedures, or additional evaluation when needed. During live video consultations, Dr. Stern can prescribe medications when appropriate, coordinate laboratory testing if appropriate, and recommend local procedures such as nail biopsy when indicated.

Nail Lichen Planus Treatment Options

Your individualized treatment plan will depend on the severity of your nail disease, the number of nails involved, whether your skin or joints are also affected, and any previous treatments you have tried.

Possible treatment approach:

  • Prescription topical medications

  • Intralesional corticosteroid injections when appropriate. Dr. Stern can guide your local provider with the best regimen for this.

  • Treatment of associated skin or scalp lichen planus

  • Laboratory testing

  • Nail care recommendations to minimize trauma

  • Monitoring nail growth and treatment response over time

  • Local care coordination with your dermatologist when needed

Explore Options for Persistent Nail Lichen Planus

If your nail lichen planus has not improved despite topical medications, steroid injections, oral medications, or repeated treatment attempts, Dr. Stern will carefully review your diagnosis, treatment history, and response to previous therapies to determine why improvement has been limited. Treatment failure may occur because nail lichen planus was mistaken for another condition, a fungal infection was present at the same time, the medication did not adequately reach the site of inflammation, the wrong treatment was used, or the treatment was not continued long enough to allow healthy nail growth. Dr. Stern will develop an individualized treatment strategy based on your specific nail findings and overall medical history.
