Nail Picking Treatment
Nail picking (onychotillomania) is a body-focused repetitive behavior that can cause significant damage to the nails, cuticles, and surrounding skin. Repeated picking may lead to chronic inflammation, infection, permanent nail deformity, and impaired nail growth. Through a virtual nail consultation, Dr. Dana Stern can evaluate the extent of nail damage, identify any associated nail disorders or infections, and develop a personalized treatment plan to restore healthy nail growth.
What Is Nail Picking?
Nail picking, medically known as onychotillomania, is a body-focused repetitive behavior (BFRB) characterized by repeatedly picking, pulling, scratching, or manipulating the nails, cuticles, or surrounding skin. Although many people are unaware they are doing it, chronic nail picking can result in damage to the nail matrix, leading to abnormal nail growth, chronic inflammation, pigment changes, scarring, and recurrent infections. In severe cases, permanent nail deformity may occur.
What Causes Nail Picking?
Nail picking is considered a body-focused repetitive behavior and may occur consciously or unconsciously. Many patients report picking during periods of stress, anxiety, boredom, or concentration, while others describe it as an automatic habit. Common contributing factors include:
Stress
Anxiety
Perfectionism
Boredom
Habitual behavior
Obsessive-compulsive tendencies
Other body-focused repetitive behaviors
Underlying skin irritation around the nails

How Dr. Stern Evaluates
Nail Picking Virtually
For both asynchronous e-visit and live video consults, Dr. Dana Stern will review your photos and intake forms, prior treatments and your current medications and health history. For asynchronous e-visits, you will receive a detailed treatment plan to share with your local healthcare provider, including recommendations for additional evaluation when needed. For live video consultations, Dr. Stern can prescribe medications, recommend treatments for damaged nails or infections, and provide individualized recommendations to promote healthy nail recovery.
Nail Picking Treatment Options
Your treatment plan will depend on the severity of nail damage, whether infection is present, and the underlying factors contributing to nail picking. Possible treatment approach:
Treatment of bacterial or fungal infection if present
Prescription medications when appropriate
Cuticle repair strategies
Barrier repair and moisturizing recommendations
Protective nail care techniques
Behavioral modification strategies and habit-reversal therapy
Referral to behavioral health specialists when appropriate
Monitoring healthy nail regrowth over time
When Nail Picking Treatment Has Not Improved
If your nails remain damaged despite attempts to stop picking or previous treatments, Dr. Stern will carefully review your nail findings and medical history to determine whether another nail disorder is contributing to the problem. Chronic nail picking may coexist with nail psoriasis, eczema, fungal infection, or chronic paronychia, all of which may require additional treatment. She will develop an individualized plan designed to restore nail health, reduce inflammation, and prevent further damage.
